Mask Exemption Form

The recent Salt Lake County Health Order, 2022-1 (effective January 8, 2022) states that “all individuals over the age of two years who can medically tolerate wearing a respirator (or until you are able to obtain a respirator, any well-fitting mask or face covering), regardless of COVID-19 vaccination status or past COVID-19 infection, must wear a respirator, masks or face covering” in public.

Vanguard Academy recognizes that some students may have a medical condition, mental health condition, or disability that prevents wearing a face covering. In order to receive an exemption from the State’s face covering requirements, this form must be completed and returned to the Attendance Office at Vanguard prior to your student attending school without a face covering.

The current Health Order (2022-1) does not give schools the ability to require the signature of a physician for a mask exemption.
